Required Documents for Training or Lessons

Before you bring your horse here for training, or before you start lessons with us, we ask that you please fill out the necessary forms. These are for everyone's protection: these forms are required by our insurance company. Please choose which forms fit the business you plan to do with us: Client Form. This is for all prospective clients of Scott Silveira Horses. Hold Harmless. This is for anyone who wants to take lessons. Training Hold Harmless. This is for anyone who wants to bring a horse for training.
